Over the past 20 years of diverse inpatient and outpatient nursing experiences in the healthcare field with emphasis not only on research, primary prevention, internal medicine, surgery, but also on tertiary care, I have gained in excellent critical thinking abilities, diagnosing and treating patients as well as astounding interpersonal and organizational skills from nursing provider's perspectives. After I graduated from a nurse practitioner school in 2017, I successfully passed the ANCC national board certified exam in early 2018. I was employed with Team Health since I graduated and working at the varieties of post acute (LTAC) settings with direct patient care till June 2022. I also continue work as a NP on as needed weekends with MidWest team for caring LTAC population till now. The LTAC settings are fast paced with involving multidisciplinary teams with caring for varieties of complex patients, and I have enjoyed this job since day one. I decided to explore a different NP role in 2022, so I tried a different career pathway to work full time at Medpace as cardiology/hybrid role of ACP (advance clinical practitioner) at Medpace in 2022-2023. But I realized that I still enjoy providing direct patient care among adult geriatric population. So I decided to re-join a full time clinical NP to provide and engage community health. I have joined Oak Street Health Primary Care since July 2023, and I have been enjoyed providing and engaging local community health. I currently follow the aspirational goals of focus on patients' outcome of their health and prevention thru Oak Street Health model which align completely with mine. I will continue paced myself to devote the best to all patient care to the next levels. I hope continue to use evidence based and advance skills to help local Cincinnati community in needs.
Provide long-term acute care (LTAC) to patients
Manage a census of patients in the inpatient LTAC hospital setting
Collaborate with physicians regarding diagnosing, treating, and providing education on the management of acute and complex illnesses and diseases in post-acute hospitalized patients
Order and interpret appropriate diagnostics, prescribe medications, and request referral from consultants, when needed
Collaborate with other disciplines and hospital-based service lines
Responsible for analytical decision-making that deals with a variety of complex situations as an independent healthcare provider on a daily basis
Work with a multidisciplinary team, including RN staff, and speech, occupational and physical therapy, dietitian, social workers, and RN utilization review staff, for discharge planning and daily progression of care
Manage an average of post-acute patient of 12-15 patients on the weekends
Take on-call schedules as needed on weekends in a post-acute hospital
Run code or rapid triage and treatment (RRT) for any urgent issues that arise
Coordinate discharge planning and treatment for patients transferring to the next facility
Provide patient care, performed assessments, and interventions for CCU and post CTICU critical ill patients

I considered myself a dedicated, genuine, well cultured, team oriented, innovative, and open-minded person. First time I came to United States in 1991 landed in Wichita, Kansas and majored in Health Science. I fell in love with the advanced and evidence-based American healthcare culture and medicine immediately. I decided to pursue in nursing degree after I finished my bachelor degree in 1995. Then I moved to Richmond, Kentucky, and enrolled at Eastern Kentucky University. I had many wonderful professors, classmates, and mentors guided me thru nursing school years. I graduated nursing degree in December 1998 as BSN. I started my first job immediately in January 1999 as an ICU RN at University of Kentucky Medical Center. Being a new ICU RN was challenge at that time, but I learned to manage the stress as well as how to prioritizing treating critical ill patients. I have learned so much from my first job and enjoyed being a nurse since. Over the past 18 years, I have worked in a variety clinical settings (both inpatient and outpatient). I enjoyed the most in teaching patients, family members as well as coworkers. I also love to learn new information from professional journals and current evidence-based information. Nursing has been a very rewarded career to me. Patients, providers, and peers all taught me different things thru my nursing careers, and many of these experiences and memories were joyful, and unforgettable.
In early 2016, I decided to advance to a higher nursing degree to broad my scope of practice in order to provide diverse and comprehensive services to needed adults as well as aging population. I worked as a RN while I chose to study the nurse practitioner program to demonstrate that I can be a goal driven and well time managed person. I successfully challenged myself and finished the Primary Care Adult Gerontology Nurse Practitioner degree from Walden University in 2017 and passed the ANCC AGPCNP-BC exam.
From NP career perspective, I have worked as a post acute (LTAC) nurse practitioner with a group of hospitalists (from full-time to prn weekends) since 2018. This field has been busy and challenged but learning how to efficient prioritize and communication with patients, families and other colleagues. It has been fun, challenging critical thinking skills. I have utilized the advanced practice skills authorized to diagnose illness, treat conditions, and provide evidence based health education to my patients as well as family members in current settings. I have taken a clinical break for a year (2022-2023) and have worked at CRO (contract research organization) as ACP at Medpace. I have learned so much from disease process, biochemistry and complex disease with future drugs/device opportunities. I find myself still missing to work in clinical settings among adult geriatric population. Fortunately, I applied Oak Street Health in 2023, and I have enjoyed working with my colleagues as well as staff to serve local community people. I will continue use evidence based knowledge and maintain enthusiasm to achieve primary health care settings.
