  • Work to develop the Innovaccer strategies and protocols from the ground up. To include working closely with the National Continuing Care Team developing a strategy and protocol for Direct to Employer contracts (example: Employee/Dependent Diabetic Program for the CHI medical plan) and for Annual Wellness Visits(AWV)
  • Worked alongside our Director of Operations in an AWV intensive. Developed a strategy to identify and then auto-assign patients that needed the visit. Worked closely with the team and the CHI Saint Joseph Medical Group to schedule and do pre-visit planning for the AWV’s. The AWV completed percentage in 2023 was 29% and the end of 2024 it was 60.2%
  • Monthly strategic payers review meetings to improve the quality of outcomes (Anthem Medicare, Anthem Commercial, Humana Medicare and CHI Medical plan). Includes working with Anthem representative in reviewing high cost patients (the reasons for the cost to include office visits, outpatient procedures, inpatient and ED visits, pharmacy cost and HCC re-documentation
  • Participating in a PIE project(Lean Six Sigma) which is a structured approach to improve processes. The project included Anthem Medicare Advantage payor and the quality measure ‘Follow-up after ED visit’. The project started on January 1, 2025 and the end goal is a 65% completion rate by 12.31.25. March 2025 = 41%, April 2025 = 52% and May 2025 = 53%
